Medical fitness certificate: purpose, issuer and Form 1A The fit-to-join line in Appendix 2, the driving licence certificate under the Central Motor Vehicles Rules, and what every version needs.
Employers, colleges, transport offices and sports bodies all ask for a fitness certificate, and each means something slightly different. The common thread is a registered doctor's signed statement for a stated purpose, and one version, Form 1A, is written into law.

- Only a doctor enrolled in a State Register or the National Register may "sign or authenticate a medical or fitness certificate" (NMC Act 2019, s.34(1)(c)).
- Appendix 2 of the 2002 conduct regulations carries the fitness line: "I, Dr. ___ after careful examination of the case certify hereby that ___ on restoration of health is now fit to join service."
- Form 1A under rule 5 of the Central Motor Vehicles Rules 1989 is the statutory example for transport-vehicle licences; it is valid for one year from issue and must carry a passport photograph.
A medical fitness certificate is a certificate in which a registered medical practitioner states that a named person is fit for a stated purpose, such as the Appendix 2 line "on restoration of health is now fit to join service" in the 2002 conduct regulations or the Form 1A finding that an applicant "is medically fit/not fit to hold a driving licence". It is a medical certificate in the regulation 1.3.3 sense, so the same duties apply: the patient's signature or thumb mark, address and identification marks go on it, a copy is kept and the register entry is made.
Who can issue a fitness certificate?
A doctor enrolled in a State Register or the National Register. Section 34(1)(c) of the National Medical Commission Act 2019 reserves the right to "sign or authenticate a medical or fitness certificate or any other certificate required by any law to be signed or authenticated by a duly qualified medical practitioner" to such persons. Some certificates narrow it further: Form 1A is "to be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government". Regulation 1.4.1 puts the doctor's registration number on every certificate.
What is a fitness certificate used for?
- Return to work after illness: the second paragraph of Appendix 2, used with the leave certificate above it.
- Joining a job or a college: the employer's or institution's own format, signed as a professional certificate under regulation 7.7. A layout is in our fitness certificate for job or college.
- Driving licence: Form 1A, discussed below. Layout in Form 1A medical certificate.
- The certificates listed in Appendix 4 of the regulations, issued under the Factory Acts, the Employees' State Insurance Act, passport rules and others.
What does Form 1A require?
Rule 5(1) of the Central Motor Vehicles Rules 1989 says an application for a licence "to drive a transport vehicle shall be accompanied by a medical certificate in Form 1-A issued by a registered medical practitioner referred to in sub-section (3) of section 8" of the Motor Vehicles Act; for a non-transport vehicle a self-declaration in Form 1 is enough. Rule 5(3): the Form 1A certificate "shall be valid for a period of one year from the date of its issue" and "shall be accompanied by a passport size copy of the photograph of the applicant". The form itself records identification marks, questions on vision, hearing, night blindness and "any defect or deformity or loss of member", a declaration that the doctor has "personally examined the applicant", colour vision "tested using standard ishihara chart", and the doctor's name, designation, seal and "Registration number of medical officer". The doctor signs across the photograph.
What does a small clinic do with a fitness request?
The front desk asks what the certificate is for and which format the receiving body wants, because a college form, an employer letter and Form 1A ask different questions. The doctor examines the person, fills the form, enters it in the certificate register with the identification marks, and keeps a copy. Regulation 7.7 applies in full: a certificate that is "untrue, misleading or improper" can cost the doctor their registration. A general fitness certificate format covers the common case.
