    What the rules say

What the Central Motor Vehicles Rules say about Form 1A

Form 1A is the medical certificate under the Central Motor Vehicles Rules, 1989. Two amendments changed it: G.S.R. 401(E) in June 2020 and G.S.R. 240(E) from 1 April 2021.

    • CMV Rules 1989, rule 5(1) and proviso, as amended by G.S.R. 240(E)

Transport vehicles, and renewals at forty

An application for a licence to drive a transport vehicle must include a medical certificate in Form 1A. The 2021 proviso adds: every renewal of a licence for a vehicle other than a transport vehicle "shall be accompanied by a medical certificate in Form 1A ... in case the applicant has attained the age of forty years."

    • CMV Rules 1989, rule 5(1); G.S.R. 240(E), items 3, 48 to 50

      Everyone else signs a self-declaration

Other applicants include "a self-declaration as to the physical fitness as appended to Form 2". Form 1, the old separate declaration, was omitted from 1 April 2021, and the line in Form 1A saying the Form 1 declaration is attached was deleted with it.

    • CMV Rules 1989, rule 5(3) and 5(4), from 1 April 2021

Issued on the Portal, valid six months

"Every medical certificate issued by a registered medical practitioner referred to in sub-section (3) of section 8 of the Act in Form 1A, shall only be issued electronically on the Portal." Its validity was cut from one year to six months, with the applicant's photograph submitted on the Portal.

    • G.S.R. 401(E), 24 June 2020

      Colour vision by Ishihara chart

Item 3(b) on red and green was omitted in 2020, and entry (v) was added to the certificate: the applicant's colour vision "has been tested using standard ishihara chart and the applicant has not been found suffering from severe or total colour blindness."

    • CMV Rules 1989, Form 1A, heading

      Who may sign

The form is "to be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government referred to under sub-section (3) of section 8". Check whether your state has appointed or authorised you before you issue one.

    • CMV Rules 1989, rule 7 and Form 1A note 1; 2002 Regulations, Appendix 4(q)

      Sign across the photograph

The practitioner shall affix signature and seal to the applicant's photograph so that they "appear partly on the photograph and partly on the form of the medical certificate". Appendix 4 of the 2002 ethics regulations lists certificates for procuring a driving licence among those clause 7.7 covers.

    Questions

    Straight answers

Who needs a Form 1A medical certificate for a driving licence?

Under rule 5(1) of the Central Motor Vehicles Rules, 1989, as amended from 1 April 2021: anyone applying for a licence to drive a transport vehicle, and anyone aged forty or more renewing a licence for a vehicle other than a transport vehicle. Rules 10, 14, 17 and 18 repeat it for learner's licences, driving licences, additions and renewals involving transport vehicles.

    What happened to Form 1?

G.S.R. 240(E) of 31 March 2021 omitted Form 1 from the rules from 1 April 2021. The self-declaration of physical fitness now sits in Form 2, the licence application, under the heading "Self-Declaration as to Physical Fitness". Applicants who do not need Form 1A answer those questions themselves and sign.

Can any MBBS doctor sign Form 1A?

The form says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government, or by a person the State Government has authorised, under section 8(3) of the Motor Vehicles Act, 1988. Ask your RTO or state transport department how your state appoints or authorises practitioners before you issue one.

How long is a Form 1A certificate valid?

Six months from the date of issue. Rule 5(3) said one year until G.S.R. 240(E) substituted six months from 1 April 2021. The same amendment requires the applicant's passport size photograph to be submitted with it electronically on the Portal.

Is a printed Form 1A still accepted?

Rule 5(4) says a Form 1A certificate "shall only be issued electronically on the Portal". Use this printed sheet to examine the applicant and record your findings, then enter them online. Ask your RTO whether it also wants the signed paper copy with the photograph.

Can a person with colour blindness get a driving licence?

Since G.S.R. 401(E) of 24 June 2020, Form 1A no longer asks whether the applicant can distinguish red and green. Instead the practitioner certifies that colour vision was tested with a standard Ishihara chart and the applicant was not found to have severe or total colour blindness. The Ministry's press release says mild to medium colour blindness is no longer a bar.

This tool reproduces Form 1A of the Central Motor Vehicles Rules, 1989 as amended by G.S.R. 401(E) of 24 June 2020 and G.S.R. 240(E) of 31 March 2021, and pre-selects nothing. It is not the Portal and does not issue a certificate. Only a practitioner appointed or authorised under section 8(3) of the Motor Vehicles Act who has examined the applicant may sign it. General information, not legal advice. Sources read and checked on 26 Oct 2026; later amendments, if any, prevail.
