What the rules say
What the regulations ask for on a fitness certificate
The same 2002 regulations govern every certificate a registered practitioner signs; Appendix 2 adds the wording for fitness to rejoin duty.
Fitness is in the same recommended form
Appendix 2 of the 2002 regulations carries two statements on one form: the period of absence, and that the patient on restoration of health is now fit to join service. The join-duty option here uses that second sentence.
2002 Regulations, Appendix 2
Same identification rules as any certificate
Clause 1.3.3 applies to every medical certificate: the patient's signature or thumb mark, address and at least one identification mark, a copy kept, and an entry in the Register of Medical Certificates.
2002 Regulations, clause 1.3.3
Registration number on the certificate
Clause 1.4.1 requires the registration number on all certificates given to patients, along with prescriptions and money receipts. The form prints it under the doctor's name and beside the signature.
2002 Regulations, clause 1.4.1
Employers and institutions may have their own form
Government service, colleges, sports bodies and transport authorities often prescribe a form of their own. When one is given to you, fill that form; the contents in clause 1.3.3 still apply.
Practice, not regulation
Driving licences use Form 1-A
A medical certificate for a driving licence is Form 1-A under the Central Motor Vehicles Rules, 1989, with its own questions and a registered medical practitioner's details. It is not this form.
Central Motor Vehicles Rules, 1989, Form 1-A
An untrue certificate can cost the registration
Clause 7.7 covers certificates given for administrative purposes or courts. A practitioner shown to have signed one that is untrue, misleading or improper is liable to have their name deleted from the Register.

Questions
Straight answers
01What is a medical fitness certificate?
A certificate from a registered medical practitioner stating that, on examination, a person is fit for a stated purpose from a stated date. The 2002 regulations' Appendix 2 form covers fitness to rejoin duty after illness; employers and institutions use the same idea for joining a job or a course.
02Is a fitness certificate needed to rejoin after sick leave?
Many employers ask for one after a longer absence, and government leave rules often require it. The Appendix 2 form combines the leave certificate and the fitness certificate on one page for that reason.
03What does 'fit for employment' cover?
Whatever the employer's form asks; when it specifies tests or conditions, use that form. This tool prints a general statement of fitness on the date of examination, with space for remarks, and does not list examinations you did not do.
04Can I issue a fitness certificate for a college admission?
Yes, if you are a registered medical practitioner and the institution accepts a practitioner's certificate. Several institutions require a medical officer of a government hospital or a specific proforma; check the admission notice before the student travels.
05Which registration number goes on the certificate?
The number from the State Medical Council or the Indian Medical Register, as required by clause 1.4.1. Add the council's name so the reader can verify it.
06Does the tool store patient details?
Only in this browser. Nothing is sent to Vitels. Use Clear after printing if the device is shared.
This page summarises the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 as amended and names other forms where they exist. It is general information for registered practitioners, not legal advice.
