What the rules say about a clinic receipt
One line in the 2002 ethics regulations, and two points from the GST law that our guide to GST on doctor consultation fees covers in detail.
- 2002 Regulations, clause 1.4.1
Registration number on every money receipt
Every physician shall display the registration number accorded by the State Medical Council or the Medical Council of India in the clinic and in all prescriptions, certificates and money receipts given to patients. This tool prints it under the doctor's name and beside the signature.
- Notification 12/2017-Central Tax (Rate), Sl. 74; CGST Act, section 23(1)(a)
Consultation fees are exempt from GST
Health care services by a clinical establishment or an authorised medical practitioner are exempt, with no fee cap. A clinic that supplies only exempt services is not liable to register. One taxable line, such as products sold over the counter, changes that.
- CGST Act, 2017, section 31(3)(c)
Registered under GST? A bill of supply
A registered person supplying exempted services issues a bill of supply instead of a tax invoice, and taxable supplies get a tax invoice. This tool makes a plain fee receipt and does not calculate tax, so a registered clinic should check its bill with its CA.
What a consultation receipt should carry
Clause 1.4.1 requires the registration number. The other items are good practice: they let the patient, and your accountant, match every receipt to a payment.
Clinic name, address and phone number
Doctor's name and recognised qualifications
Doctor's registration number, with the council that issued it
Receipt number, from one unbroken series
Date of payment
Patient's name, age and sex
Each service on its own line: consultation, procedure, dressing
Amount against each line and the total in figures
Total in words, so the figure cannot be altered
How it was paid: cash, UPI or card, with the reference for UPI and card
Signature of the doctor or the person who received the money
Billing every visit at the desk?
Vitels bills the consultation fee at the front desk, records cash, UPI or card, and prints an A5 receipt on the clinic letterhead, with a day book of revenue by doctor for your CA. Works offline. No licence fee.
Straight answers
Is there a fixed clinic receipt format in India?
The Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 ask for the doctor's registration number on every money receipt (clause 1.4.1). If your clinic is registered under GST, the bill of supply and tax invoice rules apply on top; our guide to GST on doctor consultation fees covers them.
Do I have to charge GST on the consultation fee?
No. Health care services by a clinical establishment or an authorised medical practitioner are exempt under serial number 74 of Notification No. 12/2017-Central Tax (Rate), with no fee limit. A clinic that earns only consultation and treatment fees does not need GST registration. If it also sells something taxable, the registration threshold applies; the GST guide linked below explains how it is counted.
My clinic is registered under GST. Can I use this receipt?
Registration does not make the consultation taxable, but it changes the paperwork: under section 31(3)(c) of the CGST Act a registered person issues a bill of supply for exempt services, and taxable products or services need a tax invoice. This tool makes a plain fee receipt and does not calculate tax. Ask your CA what your bill must show.
How is the amount written in words?
In the Indian system, with thousand, lakh and crore. ₹1,25,000 prints as "Rupees One Lakh Twenty Five Thousand Only", and paise are added when the total has them. Amounts in the table use Indian digit grouping.
A5 or A4?
A5 fits a consultation and a few charges and saves paper. A4 leaves room for a longer list of procedures. Choose the size here and the page prints at exactly that size.
Does the tool store patient details?
No. The patient's name and the charges stay only on this page and are gone when you close or reload it. The clinic letterhead is kept in this browser, unencrypted, so it is there next time; it is shared with the prescription and certificate makers. Nothing is sent to Vitels.
Why is this free?
Vitels is clinic software that bills every visit at the front desk and prints the receipt on the clinic's own letterhead. If you write a few receipts a day, this page is enough. If the desk bills all day, the product records cash, UPI and card against each visit and totals the day by payment mode.
This tool only lays out a receipt. It does not check the amounts, calculate GST or keep a record of what you print. This page summarises clause 1.4.1 of the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 and the GST provisions set out in our guide; it is general information, not legal or tax advice. Ask your CA how your clinic should bill.
