    What the rules say

What the rules ask for on a job or admission fitness certificate

The 2002 regulations set what every certificate must carry. The employer's or college's own rules decide the form and, for many government posts, who may sign.

    • U.P. Financial Handbook Vol. II, Chapter III, rule 10

The employer's or college's form comes first

Uttar Pradesh's rules under Fundamental Rule 10 open with: unless any other form is prescribed in the rules regulating recruitment to a particular post, the certificate shall be in the general form. Most employers and colleges work the same way. If the candidate brings a form, fill that one.

    • U.P. Financial Handbook Vol. II, Chapter III, rule 10

What a general fitness form certifies

The Uttar Pradesh general form has the examiner certify that they cannot discover any disease (communicable or otherwise), constitutional weakness or bodily infirmity, except those named, and record the candidate's age by their own statement and by appearance. This tool prints a plainer statement and leaves the findings to you.

    • U.P. Financial Handbook Vol. II, Chapter III, rules 12 and 14

Government posts may need a civil surgeon or board

In Uttar Pradesh the certificate for government service is signed by the district civil surgeon, on the appointing authority's written request, with listed exceptions. Central and other state posts have their own recruitment rules. Ask the candidate for the appointment letter before you examine.

    • 2002 Regulations, clause 1.3.3; U.P. Financial Handbook Vol. II, Ch. III, rule 10, note

      Signature taken in your presence

Clause 1.3.3 requires the candidate's signature and/or thumb mark, address and at least one identification mark on every medical certificate, a copy kept and a register entry. The Uttar Pradesh note also has the examining officer obtain the candidate's signature in their presence.

    • 2002 Regulations, clause 1.4.1

      Registration number on the certificate

Every physician shall display the registration number accorded by the State Medical Council or the Medical Council of India on all prescriptions, certificates and money receipts. Add the council's name so the employer or college can verify it.

    • 2002 Regulations, clause 7.7

A certificate is only as good as the examination

A practitioner shown to have signed a certificate for administrative purposes that is untrue, misleading or improper is liable to have their name deleted from the Register. Certify fitness only on findings you recorded.

What a job or college fitness certificate carries

The contents required by clause 1.3.3, plus what the employer or college needs to know what you examined the candidate for.

    1. Serial number from your Register of Medical Certificates, and the date and place of issue

    2. Candidate's name, age, sex and address

    3. Candidate's signature or thumb impression, taken in your presence

    4. At least one identification mark

    5. The purpose: employment and the post, admission and the course, or the sport or event

    6. Name of the employer, college or organiser

    7. Date of examination

    8. Findings you recorded: height, weight, blood pressure, pulse, vision, anything else the form asks

    9. Any condition or finding the reader should know, in the remarks

    10. Your name, qualifications, registration number and council, and your signature

    11. A copy filed and the register entry made

Findings you can look up later

Vitels keeps each patient's visits and prescriptions in one record, with prescriptions printed on your letterhead with the registration number, so an employer's query months later has an answer. Vitels itself does not print fitness certificates; this free tool does.

    Questions

    Straight answers

What is a medical fitness certificate for a job?

A certificate from a registered medical practitioner that, on a stated date of examination, the candidate was medically fit for the post named. Employers ask for it before joining. When the employer's offer letter or recruitment rules give a form, use that form; this tool is for employers that accept a practitioner's certificate on letterhead.

Can a private MBBS doctor issue a fitness certificate for a government job?

It depends on the recruitment rules for that post. In Uttar Pradesh, for example, the rules under Fundamental Rule 10 have the certificate signed by the district civil surgeon on the appointing authority's request, with exceptions. Many central and state posts use a medical board. Read the appointment letter before examining the candidate.

Which tests should a pre-employment fitness certificate include?

Whatever the employer's form or policy lists. The 2002 regulations do not prescribe examinations for a fitness certificate. This tool has blank lines for height, weight, blood pressure, pulse, vision and other findings, with no values filled in; you write only what you examined, and empty fields print as lines to complete by hand.

What does a college admission fitness certificate need?

The college's own proforma, if the prospectus or admission notice gives one. Some institutions ask for a government medical officer. Where the college accepts a practitioner's certificate, this tool prints the course and institution, the date of examination and your findings, with the contents clause 1.3.3 requires on every certificate.

How is this different from the fitness certificate for joining duty?

Fit to join duty follows illness or leave and refers back to the sick leave certificate; use the fitness certificate maker for that. This page is for a first examination before a job, a course or a sport, so it names the employer or institution and records the examination findings.

Does the tool store the candidate's details?

Patient details are kept only for this browser session, unencrypted, on this device; closing the tab or pressing Clear removes them. The letterhead stays for next time. Nothing is sent to Vitels, so as the examining practitioner you are responsible for what is typed here. Press Clear after printing on a shared computer.

This tool only lays out a form. It does not check who is using it or whether the contents are true. Only a registered medical practitioner who has examined the candidate may sign the printed certificate, and the practitioner alone is responsible for what it states (clause 7.7). This page summarises the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 as amended and, as an example of government recruitment rules, Chapter III of the Uttar Pradesh Financial Handbook Vol. II; it is general information, not legal advice.
