Clinic registration Assam: CE Act 2010 guide Who must register, the district authority, fees by area, and the rate list rule.
Clinic registration in Assam runs under the central Clinical Establishments Act, 2010, which the state has adopted, and the Assam Clinical Establishments (Registration and Regulation) Rules, 2016. Every clinic, single-doctor OPDs included, registers with the District Registering Authority. An urban OPD clinic pays ₹500 for provisional and ₹1,000 for permanent registration.

- The Assam rules of 2016 say they are "applicable to all the Clinical Establishments in the State"; the Act's definition includes a clinic run by "a single doctor".
- Each district's registering authority is chaired by the Deputy Commissioner, with the Joint Director of Health Services as convenor (notification of 3 Nov 2016).
- Rule 16(5) asks clinics to display their rates at the reception counter in the local language and English, and to send the rate list to the authority by 1 June every year.
To register a clinic in Assam you apply to the District Registering Authority of your district under the Clinical Establishments (Registration and Regulation) Act, 2010 and the Assam Clinical Establishments (Registration and Regulation) Rules, 2016. You get provisional registration first, without an inspection, and move to permanent registration once your category's minimum standards are met. Fees depend on whether the clinic is in a rural, urban or metro area. Below: who is covered, the steps, the fee table, the desk duties Assam adds, and the penalty for skipping registration.
Which law applies to clinics in Assam?
The central Clinical Establishments Act, 2010. It applies to a state that adopts it by resolution under article 252 of the Constitution (section 1(2)), and the Ministry of Health's portal lists Assam among the 19 states and union territories that have adopted it. The state's rules were made under section 54 by notification no. HLA.1652/2003/Pt-IV/468 of 14 October 2016, published in the Assam Gazette Extraordinary on 18 October 2016. On 3 November 2016 the state set up the Assam State Council for Clinical Establishments and a District Registering Authority for each district. We did not find the adoption date in a primary source we could open on 31 Oct 2026, so we have left it out. The states list shows where Assam sits nationally.
Does a single-doctor clinic have to register in Assam?
Yes. Section 2(c) of the Act covers any clinic offering diagnosis or treatment "in any recognised system of medicine" and includes one owned or managed by "a single doctor"; only Armed Forces establishments are left out. Section 11 says no one shall run a clinical establishment "unless it has been duly registered". The Assam rules' category table puts a clinic and a polyclinic of a group of practitioners under out-patient care, and the application form starts with the name of the establishment or of the doctor "in case of single practitioner". Government clinics register too: the proviso to rule 10(1) exempts them from the fee only.
If you offer services in more than one category, rule 5(3) asks for a separate provisional or permanent registration for each. A laboratory or diagnostic centre that is part of an establishment giving out-patient or in-patient care needs no separate registration.
How do you register a clinic in Assam?
- Fill Form I of the 2016 rules with the clinic's details, services, staff and infrastructure.
- Submit it to the District Registering Authority "either in person, or by post, or through web based online facility" (rule 5(1)). The Additional Chief Medical Officer of the district is the authority's member secretary (rule 4(3)), so the district health office is the place to ask.
- Pay the fee by demand draft or online transaction in favour of the registering authority (rule 10(3)). Keep the acknowledgement; an online application generates one automatically (rule 6).
- Receive the provisional certificate. The authority makes no enquiry before granting it (rule 7; section 16(1)), and section 15 of the Act sets ten days from the application.
- Display the certificate "prominently at the reception / entrance" (rule 8).
- Apply for permanent registration with evidence that you meet the minimum standards and personnel norms for your category (rule 5(2)). The authority publishes your details for 30 days for objections before granting it (rule 15(3)).
The state's health department service pages and the online portal they pointed to did not open for us on 31 Oct 2026, so we have not listed an official document checklist. Ask the district health office for the current list, and see our general documents checklist.
How long is registration valid, and what are the fees?
Provisional registration is valid to the last day of the twelfth month from issue and is renewable (section 17). A permanent certificate lasts five years (section 30(2)); apply for renewal within six months before it expires (section 30(4)). If you renew late, rule 14(1) adds "a penalty of Rs. 100 per day" to the renewal amount. The fees below are from the Assam schedule on the Ministry's portal, read on 31 Oct 2026.
| Registration or category | Validity | Rural | Urban | Metro |
|---|---|---|---|---|
| Provisional, OPD or single-doctor clinic | To the end of the 12th month; renewable | ₹100 | ₹500 | ₹1,500 |
| Permanent, OPD or single-doctor clinic | 5 years | ₹500 | ₹1,000 | ₹3,000 |
| In-patient care, 1 to 30 beds (provisional / permanent) | As above | ₹1,000 / ₹2,000 | ₹1,000 / ₹3,000 | ₹3,000 / ₹5,000 |
| In-patient care, 30 to 100 beds (provisional / permanent) | As above | ₹1,000 / ₹5,000 | ₹2,000 / ₹5,000 | ₹5,000 / ₹10,000 |
| Laboratory or testing centre (provisional / permanent) | As above | ₹500 / ₹2,000 | ₹500 / ₹3,000 | ₹1,000 / ₹5,000 |
| Appeal to the State Council | File within 30 days of the order | ₹2,000 | ₹2,000 | ₹2,000 |
The schedule does not print a renewal or duplicate-certificate fee; ask the district authority. A change of owner or management must be reported in writing within one month, with the fee, for a revised certificate (rule 12). See clinic registration fees by state for other states.
What must the OPD desk display and keep in Assam?
- Rates at the reception counter. Rule 16(5) asks for the rates for each service "at the reception counter both in the relevant vernacular" (the rule names Assamese, Bengali and Bodo) and English, and for the list to be sent to the registering authority "by 1st June of every year".
- The doctor's registration number. Rule 16(8) asks every doctor to display the registration number allotted by the State Medical Council or the Medical Council of India in the clinic and on prescriptions, certificates and money receipts; rule 16(7) asks for name, qualification and registration number on every prescription and certificate.
- Records for at least three years (rule 16(2)), and three-monthly reports of health information and statistics to the district authorities (rule 16(1)).
- Bio-medical waste authorisation from the State Pollution Control Board; see our bio-medical waste guide.
What is the penalty for running an unregistered clinic in Assam?
Section 41(1) of the Act sets a monetary penalty of up to ₹50,000 for a first contravention, up to ₹2 lakh for a second and up to ₹5 lakh for any later one. Anyone who knowingly works in an unregistered clinic faces up to ₹25,000 (section 41(2), repeated in rule 19(2)). Since 22 June 2026, under the Jan Vishwas (Amendment of Provisions) Act, 2026 and notification S.O. 3273(E), the other fines in sections 40, 43 and 46 are penalties decided by the authority; the section 41(1) amounts are unchanged.
