MRD meaning: the medical records department What IPHS 2022 and NABH say the department does, and what a small clinic keeps instead.
MRD is the department a patient or lawyer goes to when they want a hospital's file. Here is what the Health Ministry's standards and NABH say it does, and how a one-doctor clinic covers the same ground without a department.

- IPHS 2022 expands MRD as Medical Record Department and marks a Medical Record Room (MRD) as essential in sub-district and district hospitals.
- IPHS 2022 says medical record staff handle "collection, deficiency checking, coding and indexing, analysis and statistical reporting, filing, numbering, storage and retrieval of records".
- NABH's 6th edition hospital standards ask the organisation to retain clinical records "according to its requirements" and to destroy them only under written guidance.
MRD, the medical records department, is the unit of a hospital that collects, checks, codes, files, stores and retrieves patients' medical records, and issues copies and certificates from them. When a ward discharges a patient, the file goes to the MRD. When a patient asks for copies, or a court or insurer asks for a record, the MRD finds it. Some hospitals say medical record room or medical records section for the same unit.
What does an MRD do?
It looks after the record once the care team has written it. The Health Ministry's Indian Public Health Standards (IPHS) 2022, Volume I, for sub-district and district hospitals, says the Health Information Management Professional, Medical Record Analyst or Medical Record Assistant is "required for collection, deficiency checking, coding and indexing, analysis and statistical reporting, filing, numbering, storage and retrieval of records. They are also custodians of medico-legal documentations and responsible for coordinating issue of birth/death/medical certificates and notification of communicable/notifiable disease cases."
On the room itself, IPHS asks for "a proper system of collection, sorting, filing, indexing and storage of documents along with quick retrieval", notes that this is "a mandatory requirement" in light of the Right to Information Act, 2005, and says "Digitization of records wherever possible should be done while adhering to the data security guidelines of GoI."
Where does the term come from?
Public and accreditation standards. IPHS 2022 Volume I lists "MRD Medical Record Department" in its abbreviations, and its service table marks "Medical Record Room (MRD)" as essential (E) for every hospital size it covers, from the 100-bed sub-district hospital to the 500-bed district hospital. Its staffing tables include a Medical Records Officer. IPHS Volume III, for primary health centres, expands MRD as "Medical Record Room" and marks "Medical Records" as essential at PHCs.
The NABH Accreditation Standards for Hospitals (6th edition, effective 1 January 2025) list "MRD Medical Records Department". Its information management chapter says "The organisation shall maintain a complete and accurate medical record for every patient" and standard IMS.6 asks that "The organisation retains patient's clinical records, data and information according to its requirements", with destruction "in accordance with the written guidance". The Health Ministry's EHR Standards for India (2016) note in passing that "Coders usually work in medical records departments".
| Task | Hospital MRD (per IPHS 2022) | Single-doctor clinic |
|---|---|---|
| Filing and numbering | "filing, numbering" | One record per patient, found by name or phone |
| Retrieval | "storage and retrieval of records" | Pull up past visits during the consultation |
| Copies on request | Custodian of records and medico-legal documents | Copies within 72 hours under clause 1.3.2 |
| Certificates | Coordinates birth, death and medical certificates | Register of medical certificates under clause 1.3.3 |
Does a single-doctor clinic need an MRD?
Not as a department; the doctor's own duties are set by the 2002 conduct regulations. Clause 1.3.2 says a request for medical records from the patient, an authorised attendant or legal authorities shall be acknowledged and "documents shall be issued within the period of 72 hours". Clause 1.3.3 asks for a register of medical certificates, and clause 1.3.4 says "Efforts shall be made to computerize medical records for quick retrieval." Our medical record entry covers the 3-year rule and the Appendix 3 format.
What it means for a small clinic
In a clinic the doctor and the front desk are the MRD. Take a patient who asks for copies of last year's visits for an insurance claim. The desk finds the patient, prints the visit notes and prescriptions with dates, logs the request, and hands them over well inside 72 hours. A clinic with paper files spends that time searching cupboards.
Vitels keeps each patient's visits and prescriptions together, isolated per clinic, and the clinic admin can export everything as CSV at any time. See EMR software.
