Patient complaint register for a clinic. What the Patients' Rights Charter asks, and a format that fits one page.

A complaint that is written down is a complaint the clinic can answer. One that is argued at the desk and forgotten comes back as a Google review, a consumer case or a call from the district authority. The register is the cheap end of that; this is what goes in it and what the charters ask of it.

In short
  • The NHRC charter asks every clinical establishment for an internal grievance officer, acknowledgement within 24 hours with a registration number, and a written outcome within 15 days.
  • The 2021 NCCE charter asks for the officer's name and contact displayed in the local language and English, a record of grievances and action taken, and the district registering authority's details displayed.
  • A one-page register with eight columns covers all of it: number, date, patient, contact, complaint, taken by, action, closed on.
  • The desk logs; the owner or doctor decides. Nobody at the desk argues a complaint; they write it down and say when the clinic will reply.

A clinic complaint register is a dated, numbered record of every grievance a patient raises, who took it, what the clinic did and when it was closed. The NHRC Charter of Patients' Rights, which the Ministry of Health shared with every state in 2019, asks every clinical establishment to name an internal grievance redressal officer, acknowledge a complaint within 24 hours with a registration number, and give the outcome in writing within 15 days. The 2021 charter approved by the National Council for Clinical Establishments adds that the officer's name and contact details, and the district registering authority's, are displayed in the local language and English, and that records of grievances and action taken are kept. Whether any of it binds your clinic depends on your state; the register is worth keeping either way.

What do the Patients' Rights Charter and the clinic rules ask?

Two documents, both on the Ministry of Health's Clinical Establishments portal. The NHRC Charter of Patients' Rights lists, as right 17, the right to be heard and seek redressal: patients "have the right to receive in writing the outcome of the complaint within 15 days from the date of the receipt of the complaint", and "all complaints must be registered by providing a registration number". Its implementation section recommends that every clinical establishment set up an internal grievance mechanism with an "Internal Grievance Redressal Officer" who acknowledges receipt "within 24 hours by assigning a registration number", and that a patient not satisfied at the clinic may go to the district registering authority under the Clinical Establishments Act, 2010 or the state equivalent.

The Charter of Patients' Rights and Responsibilities as approved by the National Council for Clinical Establishments (updated 23 August 2021) carries it into the minimum standards as right (xix): every hospital "shall have/establish a time bound Grievance redressal mechanism", a grievance redressal officer "will be identified" and "his name and contact details will be displayed at a conspicuous place in local language and in English", "the records of grievances received and remedial action taken will be maintained", and the district registering authority's name and contact details "will also be displayed".

Whether this binds a given clinic is a state question. The Ministry told the Lok Sabha on 11 February 2022 that health is a state subject and it is for each state or UT to adopt, implement and monitor the charter, display it, and set up grievance redressal. The charter guide for clinics covers which rights are already law under other acts and which states have taken the charter up. For a clinic in a state under the Clinical Establishments Act, 2010, the 2021 charter sits in the minimum standards; for others, it is the standard a consumer forum or a state council would measure the clinic against.

What does a complaint register look like?

Patient complaint register: one line per complaint
ColumnWhat goes in it
No.Running number, by year ("2026/07"). This is the registration number the patient is given
Date and timeWhen the complaint was received, however it came (desk, phone, WhatsApp, written)
Patient and contactName and mobile number; the patient's own, or the representative's with their relation
ComplaintIn the patient's words, two or three lines. The desk does not paraphrase or soften
Taken byWho received it
AcknowledgedDate and how (in person, call, message) with the registration number
ActionWhat the officer decided and did, dated
Closed on, outcome sentThe date the written outcome went to the patient, and how

A bound register with these columns, or a sheet in the software, is the whole system for a one- or two-doctor clinic. Keep a column for "feedback" separate, or a separate book: a suggestion about the fan is not a grievance and should not carry a 15-day clock.

What does the desk do when a patient complains?

  1. Listen without arguing. "Main note kar leti hoon" ends more disputes at the desk than any explanation.
  2. Write it in the register in the patient's words, with the date and time, and give the patient the number.
  3. Say who will reply and by when: the named officer (the doctor or owner in a small clinic), within the clinic's stated period, which should be 15 days or shorter.
  4. Tell the officer the same day. A complaint that sits in the book over a weekend is one the patient has already taken elsewhere.
  5. When the officer has decided, send the outcome in writing (a WhatsApp message or a letter) and record the date it went.
  6. If the patient is not satisfied, tell them where the clinic's own process ends and the district registering authority or state council begins; the 2021 charter asks that the authority's details be on the wall.

The desk's boundary is the same as everywhere else: it records and it does not decide. A complaint about the doctor's manner, a bill, a wait, a lost report: all go in the book and to the officer. A complaint that is really a records request ("mujhe apni file chahiye") is a different process with its own 72-hour clock under regulation 1.3.2 of the 2002 ethics regulations; see patient requests for medical records.

What should be on the wall?

  • The grievance officer's name and contact, local language and English (2021 charter, right xix).
  • The district registering authority's name and contact, where the Clinical Establishments Act or a state act applies (same right).
  • The clinic's own line: "Complaints are acknowledged with a number and answered in writing within 15 days."
  • The Patients' Rights Charter itself, where the state asks for it; the NHRC charter's implementation section suggests states may require all clinical establishments to display it.

The free clinic notice board tool prints a one-page notice; put the grievance line on it.

Questions

Is a complaint register mandatory for a small clinic?

It depends on the state. The 2021 NCCE charter puts a grievance mechanism and a record of grievances into the minimum standards under the Clinical Establishments Act; states with their own acts have their own rules; elsewhere the charter is a recommendation. The Ministry's 2022 reply to the Lok Sabha says adoption and monitoring are with each state. Keep the register regardless; it costs nothing and it is what any authority will ask to see first.

Should Google reviews go in the register?

A review that names a specific grievance is a complaint the clinic has received, and the sensible course is to log it and reply in writing through a channel the patient gave, not in public. Never post patient details in a public reply, and remember that the NMC's 2026 advertising guidelines prohibit soliciting or manipulating reviews.

Who should be the grievance officer in a one-doctor clinic?

The doctor or the owner. The desk cannot be the officer, because the desk is where most complaints arise. Name the person on the wall and keep the same person for a year at a time.
