What the evidence says

What is known about no-shows at Indian clinics

The calculator is arithmetic on your inputs. These are the published figures and the rules around the fixes.

  • Anagha et al., J Oral Biol Craniofac Res, 2024; Tandon et al., J Clin Diagn Res, 2016

There is no national no-show figure for small clinics

The peer-reviewed Indian numbers come from dental teaching hospitals, where treatment takes several sittings. A Kochi tertiary dental centre recorded 8.4% missed appointments over six months of 2021; a Jaipur paediatric dental department recorded 38.6% among children in 2014. Treat them as a range, not a benchmark.

  • Anagha et al., 2024, PMC10891327

Most patients who miss did not forget

Asked why they missed, patients at the Kochi centre named personal or health issues (30.7%), distance (17.2%), an inflexible work schedule (14.7%) and transport (12.3%). Forgetfulness was 4.9%, at a centre that already called patients the day before.

  • Gurol-Urganci et al., Cochrane Database Syst Rev, 2013; Prasad and Anand, Int Dent J, 2012

Reminders help, on low to moderate evidence

A Cochrane review pooled seven randomised trials with 5,841 participants: attendance was 78.6% with text reminders against 67.8% with none. The authors rated the evidence low to moderate quality. An Indian dental college study found 79.2% on-time attendance in an SMS group against 35.5% in the control group, in a small single-centre sample.

  • Vitels guide: how to reduce patient no-shows

Count a no-show the same way every week

A no-show is a booked patient who neither came nor cancelled before the session. Patients who cancelled or moved their visit are not no-shows. Rate is no-shows divided by bookings less cancellations. That is the figure to enter above.

  • Vitels guide: walk-ins versus appointments in a small clinic

A session absorbs a miss; a slot does not

In a session booked by token, a missed token moves the queue forward and the doctor's time is not wasted if walk-ins are waiting. In fixed time slots the empty slot is lost. The calculator counts every missed booking; subtract the ones a walk-in filled.

  • NMC Guidelines on Ethical Advertising and Public Communication, 06.10.2026, para 8.1(x)

    Messages stay within the rules

A confirmation sent from the clinic number with the doctor, session, date and token is a service message. Keep offers, discounts and health tips out of it; the 2026 NMC guidelines bar inducements and digital mass solicitation.

Bringing the no-show rate down

The desk habits from the full guide, in the order to try them.

  1. Count no-shows for a month before changing anything, session by session and doctor by doctor

  2. Every evening, open tomorrow's bookings and call or message each patient from the clinic number: doctor, session, date, token

  3. Ask for a reply; a "cannot come" is moved on the spot and the place goes to a walk-in

  4. Book into morning and evening sessions with a token range, not fixed minutes, so a miss moves the queue forward

  5. Keep a share of each session open for walk-ins, sized from last month's no-show count

  6. Do not open bookings further ahead than patients can remember; call far-ahead bookings two days before as well

  7. Write the follow-up date and session on the prescription, and book it at the counter before the patient leaves

  8. Call repeat no-shows once to find the cause: wrong number, a clashing session, a family member who booked without asking

  9. Re-run this calculator after a month with the new rate in the target box

Bookings and walk-ins in one queue

In Vitels the front desk books patients into each doctor's session, checks them in and reschedules or cancels in one place. Bookings from the patient app join the same queue as walk-ins, and every patient receives their token on WhatsApp and SMS, so a missed booking moves the queue forward instead of leaving the doctor idle.

Questions

Straight answers

What is a normal no-show rate for a clinic in India?

There is no reliable figure for general OPD clinics. Two peer-reviewed Indian studies, both in dental teaching hospitals, found 8.4% in Kochi in 2021 and 38.6% among children in Jaipur in 2014. Count your own rate for a month and enter that.

How is the cost of a no-show calculated?

Booked patients a day times OPD days a month gives bookings a month. That times the no-show rate gives missed visits. Missed visits times the average fee gives the fee not collected, and times the minutes per visit gives doctor time left idle. The yearly figures are twelve months of the same.

Does a missed booking always cost the clinic money?

No. If a walk-in took the place, nothing was lost. The calculator counts every missed booking at the average fee; read it as the most the clinic could have collected, and subtract the places walk-ins filled.

What should I put in the target rate?

Your own guess of where day-before confirmation, session booking and a walk-in buffer could bring the rate. The evidence says reminders improve attendance, but the size of the change differs from clinic to clinic, so the tool does not assume a number for you.

Is anything I type sent to Vitels?

No. The figures stay in this browser so they are still there when you come back, and Clear removes them.

The calculator multiplies the figures you enter; it holds no benchmark of its own. The studies quoted are Anagha et al. (2024), Tandon et al. (2016), Prasad and Anand (2012) and the Cochrane review by Gurol-Urganci et al. (2013), as read on 22 Oct 2026; all have the limits noted above. It is operational guidance for a clinic desk, not a forecast.
