Polyclinic meaning: the multi-doctor OPD in India What MoHFW's clinic standard says, how it differs from a clinic, and what it asks of a small practice.

A polyclinic is an outpatient clinic with more than one doctor. The Clinical Establishments Act itself does not use the word; the definition sits in the minimum standard MoHFW published for clinics and polyclinics under that Act.

In short
  • CEA/Clinic-008, clause 1.2: a polyclinic provides "examination, consultation, prescription to outpatients including dispensing of medicines by more than one doctor".
  • Clause 1.1 defines a clinic in the same words, with a single doctor; clause 1.3 allows minor procedures like dressings and injections that do not need observation or short stay.
  • Section 2(c) of the Clinical Establishments Act, 2010 lists hospital, maternity home, nursing home, dispensary, clinic and sanatorium, but not polyclinic.

A polyclinic is a clinical establishment where more than one doctor examines, consults and prescribes for outpatients, which may include dispensing medicines, as defined in MoHFW's Clinical Establishment Act standard CEA/Clinic-008. In practice it is an OPD shared by two or more doctors, of the same or different specialties, under one roof and one registration. Patients are seen and go home the same day; the standard does not cover keeping anyone in.

What is a polyclinic?

A multi-doctor outpatient clinic, such as a GP and a paediatrician sharing rooms and a front desk, or two physicians covering morning and evening sessions. The standard treats a clinic and a polyclinic as one category with one set of minimum standards, and the only difference in the definitions is the number of doctors: a single doctor for a clinic, more than one for a polyclinic.

Where does the definition come from?

From MoHFW's "Clinical Establishment Act Standards for Clinic / Polyclinic only Consultation", Standard No. CEA/Clinic-008, published on the clinical establishments portal.

A polyclinic may be defined as a Clinical establishment providing examination, consultation, prescription to outpatients including dispensing of medicines by more than one doctor/ general practitioner/ specialist doctor /super-specialist doctor.

CEA/Clinic-008, clause 1.2

The Clinical Establishments (Registration and Regulation) Act, 2010 does not define the word. Its section 2(c) covers "a hospital, maternity home, nursing home, dispensary, clinic, sanatorium or an institution by whatever name called", so a polyclinic falls within the Act as a clinical establishment whatever it calls itself.

Does it apply to a single-doctor clinic?

No. A single-doctor practice is a clinic under clause 1.1. It becomes a polyclinic under the standard when a second doctor sees patients there, even part-time. The same minimum standards apply either way, and the consultation room area in Annexure 1 is "multiplied accordingly by the number of consultation chambers".

What it means for a small clinic

Take Sunrise Clinic, a sample OPD where a second doctor now sits three evenings a week. Under clause 3.5 the clinic displays, in the local language, the name of each doctor with registration number, the "Fee structure of the various doctors/ specialist", timings and services. Under 5.1 each doctor is registered with a state or central medical council. The desk now runs a separate token line for each doctor and shows each doctor's fee on the board, so a patient knows the charge before the consultation. Our post on clinic, polyclinic and nursing home definitions covers where a nursing home begins, and OPD management software shows a per-doctor queue.

Questions

What is the difference between a clinic and a polyclinic?

Under CEA/Clinic-008 a clinic is run by a single doctor and a polyclinic by more than one. Both serve outpatients only, and both follow the same minimum standards.

Can a polyclinic admit patients?

Not under this standard. Clause 1.3 allows minor procedures such as dressings and injections "however not requiring observation/short stay". Keeping patients in for nursing points toward a nursing home or hospital registration.

Does a polyclinic need registration?

Where the Clinical Establishments Act or a state Act applies, yes, as a clinical establishment. Section 11 of the 2010 Act says no person shall run one unless it is registered.
