Day book meaning: the clinic's daily collection record What it is in accounting, what the Income-tax Act 2025 says, and how a clinic closes it.

The register the front desk closes every evening and the CA asks for every month. Here is what a day book is in accounting terms, where the Income-tax Act mentions it, and what a clinic's version should hold.

In short
  • NCERT's Class 11 accountancy text says special journals "are also called daybooks or subsidiary books", and calls the cash book "the book of original entry".
  • The Income-tax Act, 2025 counts day-books among "books of account", whether kept on paper, in digital form or on cloud storage (section 2(19)).
  • Medical is a "specified profession" under section 62; the books a doctor must keep are prescribed by the Board in the rules.

A day book is a book of original entry in which money transactions are written down day by day as they happen; in a clinic it is the daily list of fees received, with the total for each payment mode. Each line is one receipt: date, receipt number, patient, what the fee was for, amount and whether it came in cash, UPI or card. At closing time the desk adds up each mode and checks the cash in the drawer against the cash line.

What is a day book in accounting?

A first record of transactions, made before anything is posted to the ledger. NCERT's Class 11 Accountancy textbook (Part I, chapter 4, "Recording of Transactions-II") explains that a growing business splits its journal into special journals, and "These special journals are also called daybooks or subsidiary books." Of the cash book it says: "It is also called the book of original entry." A clinic whose income is almost entirely fees received at the desk usually keeps one combined book that does both jobs, and calls it the day book.

Does the Income-tax Act recognise a day book?

Yes, by name. Section 2(19) of the Income-tax Act, 2025, in force from 1 April 2026, says "books or books of account" includes "ledgers, day-books, cash books, account-books and other books", kept "in written form" or "in electronic or any digital form, or on cloud based storage". Section 62(1)(a) requires any person carrying on a specified profession to keep books of account, and section 62(4) lists "medical" among specified professions. Which books, what they contain and how long they are kept are left to the Board to prescribe under section 62(3). Our post on income tax for doctors covers those rules; ask your CA which register format applies to you.

What goes in a clinic day book?

  • Date, and the receipt number from the receipt the patient was given
  • Patient name, and the doctor if the clinic has more than one
  • What the fee was for: consultation, procedure, certificate
  • Amount and mode: cash, UPI or card
  • Any refund, waiver or cancelled receipt, with the reason, on its own line
  • Totals by mode at the end of the day, and the name of whoever closed it

How does a clinic close the day book?

Take Sunrise Clinic, a sample single-doctor clinic. At 8 PM the receptionist prints the day's list, counts the cash drawer against the cash total, matches the UPI total against the payments app, and notes any difference on the page before the doctor signs it. A wrong entry is not overwritten: it is cancelled with a reason and a new receipt is issued, so the trail stays complete. Our post on closing the day: cash, UPI and card has the full routine, and the monthly accounts pack for your CA shows where the day book goes next.

In Vitels the desk records each fee as cash, UPI or card, prints an A5 receipt, and the A4 day book shows the day's receipts with totals by mode. Saved amounts cannot be edited; a mistake is voided with a reason the same day and a new receipt issued. See clinic billing software.

Questions

Is a day book the same as a cash book?

Close, but not always. NCERT treats the cash book as one of several special journals, which it says are also called daybooks. A clinic day book usually lists UPI and card receipts as well as cash, so it is wider than a cash-only book.

Can a clinic keep its day book on a computer?

Section 2(19) of the Income-tax Act, 2025 counts day-books kept in electronic or digital form, or on cloud storage, as books of account. Keep monthly copies your CA can read.

Does a doctor have to keep books of account?

Section 62(1)(a) of the Income-tax Act, 2025 requires any person carrying on a specified profession to keep books of account, and medical is a specified profession under section 62(4). The rules made under section 62(3) list the books. Ask your CA how this applies to you, especially under presumptive taxation.

Should the money receipt carry the doctor's registration number?

Yes. Clause 1.4.1 of the 2002 conduct regulations asks every physician to display the registration number in all prescriptions, certificates and money receipts given to patients.
