What the rules say about an OPD register
No rule prints a register layout for you. These are the clauses that decide what a clinic records for each visit and how long it keeps it.
- NCCE standard CEA/Clinic-008, 10.1.1
Every patient is registered
The national standard for a consultation-only clinic says, in its process section: "Every patient visiting the clinic shall be registered." The daily register is the simplest proof that the desk does it.
- NCCE standard CEA/Clinic-008, 9.1
Records and quarterly statistics
Every clinical establishment "shall maintain medical records of patients treated by it and health information and statistics in respect of national programmes" and send them to the district as quarterly reports. The register's daily counts feed those figures.
- NCCE standard CEA/Clinic-008, 9.2
Kept for 3 or 5 years
"Copies of all records and statistics shall be kept with the clinical establishment concerned for at least 3 or 5 years or in accordance with CEA act rules." Number the registers by month and shelve them together.
- CE (Central Government) Rules 2012, G.S.R. 387(E), rule 9(iv), (v)
Statistics under other laws, EMR when notified
Rule 9(v) asks every establishment to "maintain information and statistics in accordance with all other applicable laws". Rule 9(iv) asks for electronic records "as may be determined and issued by the Central Government or the State Government".
- 2002 Regulations, clause 1.3.1 and Appendix 3
The patient record behind the register
The register is an index. Clause 1.3.1 keeps indoor records for 3 years in the Appendix 3 proforma: name, age, sex, address, occupation, date of first visit, clinical note, diagnosis, investigations, advice and follow-up. The New / Repeat column mirrors the date of first visit.
- 2002 Regulations, clause 1.3.2
Copies within 72 hours
When a patient, an authorised attendant or a legal authority asks for records, the request is acknowledged and documents "shall be issued within the period of 72 hours". A serial number on both the register and the prescription makes the search quick.
How to fill each column
One line per visit, written at the desk as the patient registers. The fee and mode are filled when the money is received.
S. No.: printed for you and running on across pages. Write the same number on the patient's prescription or OPD card.
Token: the number on the slip handed to the patient, so the register and the queue match.
Time: when the patient registered, in 24-hour time.
Patient name: in full, spelt as on an ID where possible, so the next visit is easy to find.
Age / Sex: age in years, or months for an infant, and M, F or O.
Phone: one number per patient. Families often share a phone, so search by name as well.
New / Repeat: circle N on the first visit to the clinic, R for every later visit.
Doctor: the doctor who saw the patient, in a clinic with more than one.
Fee and mode: the amount actually received, and tick cash, UPI or card. Write "no fee" rather than leaving it blank.
Remarks: review date, referral, or a cross-reference to the medico-legal register. Keep diagnoses out of the desk register.
At closing: add the page total by mode, match it with the cash drawer and the UPI statement, and initial the page.
Lock the filled register away at night. Names and phone numbers are patient confidences under clause 2.2.
The register, built as the desk works
In Vitels the front desk registers the patient once, and the day's tokens, visits, fees and payment mode fill the register as they happen. The clinic admin can export everything as CSV at any time. The queue and billing keep working offline; new patient registration needs a connection.
Straight answers
Is an OPD register mandatory for a clinic in India?
In states that follow the Clinical Establishments Act, the clinic standard CEA/Clinic-008 says every patient visiting the clinic shall be registered, and section 9 asks for records and quarterly statistics. States with their own Acts set their own rules. The 2002 regulations separately require medical records and copies within 72 hours. A daily register is the plainest way to meet all of these.
How long should a clinic keep the OPD register?
The clinic standard says records and statistics are kept "for at least 3 or 5 years or in accordance with CEA act rules". Clause 1.3.1 of the 2002 regulations sets 3 years for indoor records. For an OPD register, 3 years is the floor; many clinics keep them longer because records are asked for in insurance and legal matters.
What columns should an OPD register have?
This format uses serial number, token, time, patient name, age and sex, phone, new or repeat visit, doctor, fee, payment mode and remarks. That covers registration, the daily statistics the district asks for and the day's collection. The clinical note belongs in the patient's record, which follows Appendix 3 of the 2002 regulations.
Can I keep the OPD register on a computer instead of paper?
Yes. Clause 1.3.4 of the 2002 regulations says "Efforts shall be made to computerize medical records for quick retrieval." Rule 9(iv) of the Clinical Establishments (Central Government) Rules, 2012 asks for electronic records as the government determines. Whichever you use, keep backups and an export you can print.
Should the diagnosis go in the OPD register?
Not in the desk register. Front-desk staff, other patients and visitors see it through the day. The diagnosis belongs in the patient's record and prescription, where Appendix 3 and standard 10.2.1 put it. Clause 2.2 of the 2002 regulations treats what a patient tells the doctor as a confidence.
Does this tool store patient details?
No patient details are typed into it. It prints a blank register with your clinic name, the date and running serial numbers. The clinic name and doctor are kept in this browser so the other Vitels tools can reuse them, and nothing is sent to Vitels.
This page summarises the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002, the NCCE minimum standard CEA/Clinic-008 and the Clinical Establishments (Central Government) Rules, 2012, as read on 26 Oct 2026. It is operational guidance for clinics, not legal advice. States with their own clinical establishments Acts may ask for other registers or periods; confirm with your state authority or council.
